When To Let Go

In the ongoing healthcare debate, one question is often missed. When do you let go? It's a tough question that involve many factors. How much money one has? Who is left behind? What sort of pain and suffering are involved? I know what I would do if I were the one dying, but I am not sure I would advise other loved ones in my life to do the same.
